Blood is the right matrix for lead and for recent exposure to the others. It is a poorer measure of long-term arsenic exposure, where speciated urinary arsenic is the usual investigation, and blood cadmium reflects recent months rather than a lifetime burden. Collected at 35 partner centres nationwide and processed in a CAP-accredited laboratory operating to MS ISO 15189. Every report is read by a doctor registered with the Malaysian Medical Council before the e-consultation. Trace metal results are highly sensitive to collection technique and tube type, which is why the panel is collected only into the specified K2EDTA tubes and only at partner centres.

The toxicology of these five metals is long established, including the mechanisms by which each causes harm and the organ systems each affects (Tchounwou PB, et al. For lead there is no blood level demonstrated to be without effect: a large population cohort found associations with cardiovascular mortality at concentrations well inside what were once considered acceptable ranges (Lanphear BP, et al. Lancet Public Health. 2018;3(4):e177 to e184. doi.org/10.1016/S2468-2667(18)30025-2), so a lead result is interpreted as a continuum rather than against a pass mark. For arsenic, the health effects of chronic exposure are wide-ranging but the exposure is assessed primarily through speciated urinary measurement, because blood arsenic clears quickly and organic arsenic from seafood can raise a total figure without any toxicological meaning (Naujokas MF, et al. That is why the doctor asks about seafood in the days before the draw, and why a raised total arsenic is a prompt for a better test rather than a conclusion.

Without an identifiable exposure, a normal result is the overwhelmingly likely outcome, and tiredness or poor concentration is far more often thyroid, iron, blood sugar, sleep or mood. A RM126 screening covers those common causes. This panel earns its price when there is something specific to check: a workplace, a renovation, a water source, an imported cosmetic or a traditional remedy.

What happens if a level comes back raised?

The e-consultation reads it in context and plans the next step, which is usually confirming the result, identifying and removing the source, and where relevant an occupational health or toxicology referral. What it is not is an immediate reason to start chelation. Chelation carries real risk and is a specialist decision made on confirmed, clinically significant levels.

Is blood or urine the better sample?

It depends on the metal and on the timing. Blood is the standard measure for lead and is appropriate for recent exposure to the others. For arsenic, speciated urinary measurement is the usual investigation, because blood arsenic clears quickly and organic arsenic from seafood can raise a total figure with no toxicological meaning. Do I need to fast or avoid anything beforehand?

No fasting. It is worth avoiding large amounts of seafood in the two or three days before the draw, because that can raise total arsenic without indicating any harmful exposure, and worth telling the doctor about any supplements, traditional remedies or imported cosmetics you use, since several are recognised sources.

How long do results take?

Longer than a routine blood panel, and we confirm the expected timeline with you at booking rather than publishing a figure we cannot hold to. Trace metal analysis is a specialist assay. The e-consultation is scheduled once the report is ready, and if any level needs urgent attention we contact you rather than waiting for the scheduled call.

Can I use the report for a workplace requirement?

Often yes, but tell us at booking so the report is issued in the form your employer or the relevant authority accepts. Occupational surveillance sometimes specifies particular metals, particular matrices or a particular reporting format, and it is far easier to arrange that before the draw than afterwards.
