Tired Is Not A Diagnosis, But It Usually Has A Cause

Most people who are persistently tired are told to sleep more, drink water and manage stress. That advice is not wrong, but it is given before anyone has checked whether the thyroid is underactive, whether iron stores are low, or whether vitamin D and B12 have quietly dropped. Those are ordinary, common, measurable and treatable. They are also invisible without a blood test.

Iron is the one that gets missed most. Iron stores can fall far enough to cause real fatigue while a standard full blood count still reads normal, which is why a panel that checks only for anaemia can come back clear and leave you no wiser. This panel measures iron status directly, alongside a full thyroid picture rather than TSH alone. It is a screening panel and not a diagnosis: it looks for common causes, and a clear result does not mean nothing is wrong.

Thyroid
TSH The pituitary signal telling the thyroid how hard to work. Whether the thyroid is under or over active. The usual first test, but not the whole picture.
Free T4 The main circulating thyroid hormone, unbound and available. How much hormone is actually available, which a TSH result alone does not show.
Free T3 The active thyroid hormone that cells use. Whether conversion to the active form is happening normally.
Iron and nutrients
Iron Studies Iron status, measured directly rather than inferred from red cells. Whether iron is low. Stores can fall far enough to cause fatigue while a blood count still reads normal.
Vitamin D Circulating vitamin D. Whether levels are low, which is common in people who work indoors regardless of climate.
iPTH Parathyroid hormone, which regulates calcium. Read alongside vitamin D, it shows how the body is responding to a low level.
Vitamin B12 Circulating B12. Whether B12 is low, which affects energy, nerve function and red cell production.
Folate (B9) Circulating folate. Whether folate is low. It works with B12, so both are read together.
Blood, inflammation and sugar
Full Blood Examination with ESR Red cells, white cells with differential, platelets, and sedimentation rate. Anaemia, signs of infection, and a general inflammation reading.
High sensitivity CRP An inflammation protein at low concentrations. Whether low grade inflammation is present. Also rises with infection, so timing matters.
Rheumatoid Factor An antibody associated with some inflammatory joint conditions. A general inflammatory signal. Needs clinical interpretation and is not read alone.
HbA1c and Fasting Blood Sugar Average blood sugar over about three months, and glucose on the day. Whether blood sugar swings are contributing to energy crashes.
Organ function and cardiovascular, same draw
Renal Profile Kidney function markers and electrolytes. Reduced kidney function is a genuine and often silent cause of tiredness.
Liver Function Test Liver enzymes, proteins and bilirubin. Liver problems can present as fatigue long before anything else shows.
Lipid Studies, AIP, CRI-1, CRI-2 Cholesterol and fats, plus three calculated ratios. General cardiovascular picture. The three indices need no additional blood.
High sensitive Troponin T A protein released by heart muscle, at very low concentrations. Included in the base panel. In screening it is a trend marker, not an emergency test.
Urine FEME Protein, blood, glucose and sediment in urine. Urinary tract and kidney signals.

Reference ranges vary by laboratory and are printed on your report. A result outside a range is a reason to discuss it with your doctor, not a diagnosis on its own.

Always tired blood test Malaysia: what does this panel check?


It checks 21 test groups from one blood draw and one urine sample. The fatigue-specific ones are full thyroid function with TSH, Free T3 and Free T4, iron studies, vitamin D with iPTH, and B12 and folate. The rest is a complete base health screen. Every test is listed with an explanation in the chart above.
My doctor already checked my thyroid and said it was normal. Why test again?


Most routine checks measure TSH alone. This panel adds Free T3 and Free T4, the hormones the body actually uses. A TSH within range does not always mean the active hormones are where they should be, and the three are interpreted together rather than separately.
Why does iron matter if my blood count was normal?


Because they measure different things. A full blood count shows whether you are anaemic. Iron studies show how much iron you actually have in store. Stores can run low enough to affect how you feel while the blood count still reads normal, which is why a clear count is not the end of the question.
Does this test measure cortisol or stress hormones?


No, and it is worth saying plainly. Cortisol is not included in this panel. This is a panel for the physical causes of tiredness: thyroid, iron, nutrients, anaemia and blood sugar. If your tiredness is specifically about being wired and unable to switch off, message us and a doctor will tell you what would actually help.
Do I need to fast, and should I stop my supplements?


Fast for 12 hours. Plain water is fine and makes the draw easier. Tell us if you take iron, vitamin D or B12 supplements when you book, because recent supplementation changes how those results read. Some doctors ask you to pause them for a period beforehand, so ask before you stop anything.
How long do results take?


4 working days from the date of your draw. Your doctor e-consultation is arranged once all results are in, so everything is discussed together rather than in pieces as individual markers return.
What if everything comes back normal?


That happens, and it is not a wasted test. It rules out the common physical causes, which is genuinely useful information and changes what a doctor looks at next. Sleep quality, mood, medication and workload are all real causes of fatigue that no blood test measures. Your consultation covers where to go from there.
